Chronic Disease
The South Australian Chronic Disease Action Plan recognises that secondary prevention, early detection and early intervention strategies can reduce the prevalence of chronic disease, delay disease progression and improve the health and wellbeing of the population as a whole. Chronic disease prevention aims to reduce risk at all stages of the disease continuum and at all life stages.
When risk is identified early, appropriate lifestyle-based strategies can have long term positive effects on health and wellbeing. Evidence supports the use of risk assessment tools; practice based screening, risk reduction programs, and the establishment of registers and recall systems for risk factors and chronic disease.

MMGPN is committed to supporting and simplifying access to programs and services that promote prevention and early intervention.
Reset your life is a Commonwealth Government initiative for prevention of type 2 diabetes. The program targets 40-49 year olds (15-54 Aboriginal or Torres Strait Islander) and is delivered by MMGPN and accredited providers at selected sites across the Murray Mallee.
Eligibility for the program is dependent on a risk assessment using the AUSDRISK tool.
