What is a HMR?
  • Comprehensive information about the consumer and their medicine use is collated and assessed in order to identify and meet medication-related needs.
  • Medication-related problems are identified and resolved in order to enhance quality of life and optimise the benefits achieved through the quality use of medicines.
 
Aims
  • To improve the consumer’s quality of life and health outcomes by a best practice approach with collaboration between the GP, pharmacist and consumer.
  • To help establish cooperative working relationship between pharmacists and other members of the health care team in order to benefit health and well being.
  • To provide a medication information resource for the consumer and health professionals involved.
 
Who is eligible for a HMR?
  • Persons living in the community.
  • NOTE – It is not available for inpatients of hospital, day hospital facility, residents in RACFS.
 
 
INFORMATION FOR GPS
 
Guide for patient selection
Currently taking 5 or more regular medications.
Taking more than 12 doses of medication per day.
Recent discharge from a facility/hospital (in the last 4 weeks).
Significant changes made to medication regimen in the last 3 months.
On medication with a narrow therapeutic index or medications requiring therapeutic monitoring.
Symptoms suggestive of an adverse drug reaction.
Sub-therapeutic response to medication treatment.
Suspected of non-compliance or inability to manage medication-related therapeutic devices.
Patients having difficulty managing their own medications because of literacy or language difficulties, dexterity problems or impaired sight, confusion/dementia or other cognitive difficulties.
Attending a number of different doctors, both general practitioners and specialists.
Other medication issues/problems.

Only the GP can initiate the referral which must be to the Community Pharmacy nominated by the patient.
 
First Visit
Explain the HMR process and practice’s charging procedure. Obtain informed consent from patient, document in medical record.
  • Complete referral form. Include medication and medical history and recent blood tests; e.g. blood electrolytes and other bio-chemical test results.
  • Ensure delivery of referral form to patient’s chosen community pharmacy. (Patient can deliver referral form if appropriate).
  • Patient can be charged for initial consult only if attending for matter unrelated to HMR.
 
Final Visit
  • GP and the accredited pharmacist MUST discuss the report either face-to-face or over the phone.
  • Use information from pharmacist’s report to draft a medication management plan.
  • Patient recalled to discuss and agreed to the plan.
  • Patient, Community Pharmacy and GP Records ALL provided with a copy.

MBS Item Number 900 is used to charge for a Home Medicines Review. Charge this item number on final visit. Refer to Members section for details.
