Palliative Care

The Riverland Divisions of General Practice and the Murray Mallee General Practice Network Rural Palliative Care Project is facilitated nationwide by the Australian Divisions of General Practice 2008-2010.

The RDGP & MMGPN is coordinating the project with the support of key partners and the Palliative Care Service.
 
Over the next 2 years the Project will be developed and implemented in accordance with the Statewide Palliative Care Strategic Plan and the National Palliative Care Strategy.
 
The development of the Project will occur in consultation with service providers and members of the community who are sensitive to the needs of people living in our community with a life limiting illness.
 
The aim of the Rural Palliative Care Project is to improve access to quality, coordinated palliative care services for people in the Riverland and Murray Mallee regions of SA.

The Rural Palliative Care Project provides both regions with a fantastic opportunity to... ‘expand the sustainable model for healthcare delivery in a rural and remote SA which is, above all, sensitive to the needs of those living in local communities’
 
The Vision.......
  • Increased community awareness of palliative care and the support options available within local communities.
  • Effective coordinated communication processes between multidisciplinary teams, service providers, the patient and their family/caregivers.
  • The provision of palliative care education, mentor support and resource groups for GP’s, multidisciplinary teams, nursing, medical and pharmacy.
  • The coordination of multidisciplinary team meetings.
